Cancellation Policy
By signing this document, I (or my child/dependent) wish to voluntarily participate in the training program hosted by Divine Inspirations Center for the Arts (DICA). I understand that participation in dance, fitness, or movement-based programs involves physical activity and carries a risk of injury. 1. Acknowledgement of Risk I understand and acknowledge that participation in any dance or movement training may involve inherent risks, including but not limited to, physical injury, strain, or accidents. I voluntarily assume all risks related to participation. 2. Medical Authorization In the event of a medical emergency, I authorize DICA staff to seek appropriate medical care. I understand that every effort will be made to contact the parent/guardian (if applicable) and that I am responsible for any medical expenses incurred as a result of emergency treatment. 3. Release of Liability I hereby release, discharge, and hold harmless Divine Inspirations Center for the Arts, its directors, staff, volunteers, and agents from any and all claims, demands, or causes of action related to injury, illness, loss, or damage arising from participation in this program, whether caused by negligence or otherwise. 4. Media Release I grant permission for DICA to photograph, video record, or otherwise capture my (or my child’s) image during training sessions and to use such media for promotional, marketing, or educational purposes, in print and digital formats. 5. Conduct and Dismissal Policy I understand that all participants are expected to conduct themselves respectfully and safely. DICA reserves the right to dismiss any participant who does not comply with policies, safety protocols, or respectful behavior without refund. 6. Payment and Refund Acknowledgment I understand that training fees are non-refundable and non-transferable, including in cases of absence, illness, or withdrawal, unless otherwise noted in writing by DICA.
